Transferring an Account to Credential Direct

Complete the appropriate form below and send it to us. In addition, please also include a photocopy of your latest statement from your current brokerage.

Deposit or Withdraw Share Certificate

• Irrevocable Power of Attorney for Fixed Income and Security Certificates (For depositing a share certificate into your Credential Direct account. Include your share certificate. We recommend sending your certificates with a courier service that can trace your shipment.) Plus these additional forms, if required: 
  • Certificate Registered to a Non-incorporated Entity or an Association
  • Certificate Registered to One-Person Company
  • Certificate Registered to Corporation

• W-8ECI — Certificate for Exemption from Withholding on Income Connected with Trade or Business in the United States
